HDMiniCam Pro connects fine on WiFi, but just says "Logging In..." over the internet. Used to work fine

Using HDMiniCam Pro while on the same wifi network, the camera works flawlessly.  But when I leave and go over the internet, it just says "Logging in..." and I cannot connect.  I've reset the camera, reset the router, then reset the camera again, it just won't work over the internet.

Does the camera make a connection to your servers?  Or does it use uPNP?  Help me diagnose what's wrong, I support routers and networks for a living, but without knowing how the camera operates, I cannot diagnose why it won't work...
